Flu Info

Here are the flu-related documents (.pdf) you may wish to download (save to your machine) and/or print. Depending on your browser, follow these simple steps:

  • Internet Exporer (Microsoft): right click then Save Target As… You will be asked where to save the document.
  • Firefox (Mozilla): right click then Save Link As… It will be saved to your desktop.
  • Print the Request Form and fax the completed form with directions to HPA. Note that it is very important that we have 3 optional dates to choose. We will confirm the date of your flu clinic by phone or e-mail. If the clinic is 2 or 3 hours then please schedule the time either in the morning or afternoon. It is also very important to have an accurate estimate of the number of employees receiving the flu shot.
  • Make copies of the Influenza Consent Form and Flu Information for employees for those receiving a flu shot.
